INTERNAL ROOT RESORPTION: Silent but Destructive

Seen that perfectly round radiolucency in the canal? It’s not always a cyst.
Sometimes, it’s your pulp eating away from within.

What you’re seeing: A classic case of internal root resorption (IRR) — an idiopathic, often asymptomatic condition that silently threatens the integrity of the tooth.

🔍 Radiographic Clue:

* Symmetrical radiolucency within the canal
* Intact PDL & lamina dura
* No movement on angled radiographs = true internal
(A “heart-shaped” shadow is often your first and only clue!)

 

⚠️ Clinical Signs:

* Pink spot in crown (rare)
* Spontaneous pain (if pulp is still vital)
* No mobility — until it’s too late

💡 Treatment Protocol:

➡️ Immediate endodontic intervention
➡️ Use of thermoplastic obturation
➡️ MTA or bioceramics for perforations

🎯 Why it matters:

Missing IRR in its early stage = extraction risk later.
This isn’t just about tooth structure — it’s about timing.
